OVERVIEW OF PROGRAM
The program is a fully operational Behavioral Health Service Organization (BHSO) currently accepting Medicaid CHIP/Medicare and various commercial payer mix payments.
This program supports the vision of providing comprehensive behavioral health and substance use disorder treatment services to an underserved and overlooked community.
Our program is actively serving more than 100 patients and is staffed by a multidisciplinary team of licensed health professionals dedicated to supporting long-term recovery and wellness. The vision in the pipeline is to obtain a Federally Qualified Health Center (FQHC) designation which will enable the program to expand further and stronger for the community. The program will also benefit greatly from a Health Professional Shortage Area (HPSA) designation, which will also promote awareness and expansion. This is also a part of the vision and goal.
Our clinical team onsite includes:
Licensed Professional Clinical Counselors (LPCC)
Licensed Clinical Alcohol and Drug Counselors (LCADC)
Certified Alcohol and Drug Counselors (CADC)
Licensed Clinical Social Workers (LCSW)
Mental Health Advanced Practice Registered Nurses (MH-APRN)
Physician specializing in addiction medicine and has operational oversight for Medication- Assisted Treatment (MAT)
The program onsite provides integrated treatment services for individuals experiencing substance use disorders and mental health conditions. Our team's evidence-based clinical approach addresses both behavioral health and addiction needs through individualized treatment, planning and ongoing clinical support.
Patient care services include:
Comprehensive clinical assessments and evaluations
Individualized treatment plans
Individual counseling sessions
Group therapy sessions
MAT services
Mental health treatment services
Substance use disorder treatment service
Case management and recovery support
Transportation assistance to ensure access to treatment and support services
Transition of care to support the patient need and experience
Patient assistance to resources within the program and community
